SUMMARY:DAL RISCHIO ALLA PROTEZIONE: Percorso Integrato nel Paziente Nefro-Metabolico
DESCRIPTION:La crescente prevalenza sia del diabete mellito tipo 2\, che della malattia renale cronica rappresenta una delle principali sfide cliniche e organizzative per il sistema sanitario. Il paziente nefro-metabolico è oggi caratterizzato da un rischio cardiovascolare e renale estremamente elevato\, da una progressione spesso silente della malattia e da un’elevata complessità gestionale che richiede un approccio integrato e multidisciplinare.\nNegli ultimi anni\, l’introduzione di farmaci cardio-nefroprotettivi\, come Finerenone\, insieme alle terapie consolidate per il diabete e la MRC\, ha dimostrato di ridurre in modo significativo la progressione della malattia renale e gli eventi cardiovascolari nei pazienti con diabete e albuminuria. Tuttavia\, la trasposizione delle evidenze nella pratica clinica presenta criticità come: inerzia terapeutica\, timore di eventi avversi\, difficoltà di coordinamento tra specialisti e medicina territoriale\, variabilità nei percorsi assistenziali.\nIl progetto formativo nasce quindi dall’esigenza di favorire l’aggiornamento scientifico sulle più recenti evidenze terapeutiche nel paziente diabetico con CKD e albuminuria condividendo modelli organizzativi per affrontare le criticità e ottimizzare la terapia nefro protettiva promuovendo un’integrazione tra specialisti\, il tutto strutturato in due momenti un primo webinar focalizzato sull’inquadramento clinico\, sull’analisi dei gap e sulla definizione degli obiettivi\, seguito da un secondo evento residenziale dedicato al confronto operativo e alla multidisciplinarietà.\nL’obiettivo finale è tradurre le evidenze in pratica clinica\, migliorando la tempestività dell’intervento terapeutico\, l’appropriatezza prescrittiva e la protezione cardio-renale del paziente nefro-metabolico\, attraverso un modello condiviso\, replicabile e orientato agli outcome. \n  \nCON LA SPONSORIZZAZIONE NON CONDIZIONANTE DI : \n \n  \n  \n  \n  \n[vc_btn title=”Scarica la brochure” color=”primary” link=”url:brochure] \n
